Yes, left it in the ignition (with engine running)
On a chilly 14 degree morning, went out to warm up the car. Went back inside the house to finish getting ready and came out to find that the automatic locks on the car had malfunctioned and locked the car, (probably due to the frozen weather).
AAA came to save the day.

Yes, left it in the ignition (engine not running).
My girl friend left her keys in the car and it took her two hours to get out.

Yes, before we had remote control entry. It was at a time when all one had to do was push a button and shut the car door. I forgot I laid the keys in the passenger seat before gathering my things and leaving the car.

Not since my cars require me to lock driver door with a key. Except that time the piece of junk Windstar decided to lock the door for me. Got out to close a gate, left the engine running for a second, inadvertently closed the door, and that wonderful <sarcasm> feature of the Windstar decided the vehicle was in motion and locked the doors. Worst vehicle I have ever owned, but I digress.

Many years ago I had to push all locks down . My children did it automatically. Once I stepped out to check my car. the motor running and they locked it with the motor running. Now I have never done that. I lock it from my key.
